Leicester, Leicestershire and Rutland (LLR) Joint Plans
Transforming Care Plan
We plan to work with stakeholders to develop good local services and support in Leicester, Leicestershire and Rutland that will help people with a learning disability and/or autism and those who may have a mental health problem to stay well in the community. Our focus is on ensuring there is the right support to enable timely hospital discharge and providing early multi-disciplinary support for people at risk, in order to avoid admission. This will include good support for carers, including through the remodelling of our short breaks offer, the proactive implementation of personal health budgets, and working with local NHS, care providers, housing providers and the local workforce to develop services and support to meet current and future need. This will reduce our use of non-secure and low secure inpatient beds by 50%.
Future in Mind Transformational Plan
The Transformational Plan produced in November 2015 sets out Leicester, Leicestershire and Rutland’s (LLR) a multi-agency approach to improve mental health and wellbeing in children and young people (C&YP) up 25. This plan is based on principles set out in The Department of Health’s Task Force Report (Feb 2015): Future in Mind: Promoting and improving our children and young people’s mental health and wellbeing.
