Leicester, Leicestershire and Rutland (LLR) Joint Plans

Transforming Care Plan 

We plan to work with stakeholders to develop good local services and support in Leicester, Leicestershire and Rutland that will help people with a learning disability and/or autism and those who may have a mental health problem to stay well in the community. Our focus is on ensuring there is the right support to enable timely hospital  discharge and providing early multi-disciplinary support for people at risk, in order to avoid admission. This will include good support for carers, including through the remodelling of our short breaks offer,  the proactive implementation of personal health budgets, and working with local NHS, care providers, housing providers and the local workforce to develop services and support to meet current and future need. This will reduce our use of non-secure and low secure inpatient beds by 50%.

Health and Wellbeing Boards bring together those who buy services across the NHS, public health, social care and children’s services, elected representatives and representatives from HealthWatch to plan the right services for their area.
